Audit Request Form (PDF)
This form must be used to apply for an audit for graduation. You will be charged a graduation fee for the semester you plan on graduating. |
Challenge a Course (PDF)
This form is submitted to the appropriate academic division and after the challenge has been completed submitted to the Office of Registrar along with a change of registration form. |
Change of Major (PDF)
This form to be used if a student wants to add or drop their major or certification. |
Change of Name/Address/Phone (PDF)
Changes in your student data can be made using this form. You must include all of the requested information and fax, mail, or deliver to the Registar's office. We will not process incomplete requests. |
Change of Registration (PDF)
This form must be used to make any changes to a student schedule. Check academic calendar for add/drop dates. |
Duplicate Diploma Request (PDF)
This form is to be used to request an official duplicate diploma. |
New Guest Student (PDF)
This form is for guest students who do not intend to be admitted to Carlow University at a later date and may be currently enrolled at another institution. If your intent is admissions please contact the admissions office. |
Pass/Fail Option Form (PDF)
This form is to be used to explore disciplines without jeopardizing Grade Point Average. |
Permission to Enter Closed Class (PDF)
This form to be used when a class is closed and a student wants to seek permission from the instructor to enter the course. |
Pre-Registration Form (PDF)
This form can be used by students and advisors for pre-registration. If form is being brought to the Registrar's Office the advisor must sign it. If student has not already submitted a Letter of Intent, please do so before processing this form. |
Proposal for Independent Study (PDF)
This form is used if an upper level students chooses to design a course to pursue independently under the guidance of a sponsoring faculty member. |

Transcript Request (PDF)
An Official Transcript will be issued only upon the written request of the student in accordance with the Family Education Rights and Privacy Act of 1974 as amended. Requests can be made by mailing completed form or in person. |
Tutorial Proposal (PDF)
This is a course of study which duplicates the number, content, and objectives of a course presently listed in the college catalog but which is not current schedule. |
